STUDENT ACADEMIC PROBLEM

Description:
A student is struggling in school. At mid-year his language arts assessment shows that he is at the bottom of the class, making very little progress since the beginning of school. Due to his academic status, he qualifies for Title I services. At conference time, I informed his mom of his progress and the opportunity to receive additional support for success.
