Official Student Online Forms

An active Lotus Notes login is required to access and complete online forms. Former students will not be able to access online forms.

As of July 18, 2006, the College of St. Catherine authorized the National Student Clearinghouse to provide transcript ordering via the Web. The online form, "Transcript Request", is no longer available. For more information about requesting a transcript click on the following link: Ordering Transcripts

If you experience any problems with online forms, please send email to: registrar@stkate.edu. In the event the Office of the Registrar needs to contact you regarding your online request, notification will be sent by email to your LOTUS NOTES account.

A downloadable "Change in Registration" (add/drop) form is available below in PDF format for your convenience.
NOTE: Viewing PDF documents requires the free Adobe® Acrobat® Reader version 4.0 or greater. If you don't already have it installed, you will need to download and install it before using Acrobat documents, simply click on the "Get Acrobat Reader" icon below.
