Instructions for completing the evaluation:
- In the box above, enter the numeric code number given to you by your instructor, then click on the GO button, or just press ENTER on the keyboard. This will load the evaluation form.
- Please check to make sure the course and instructor information listed at the top of the evaluation form matches the course you are evaluating. If it doesn't, click the back button on your browser and double check your code number.
- Complete the evaluation by ranking your response on the scale provided. Don't forget to respond to the three written questions!
- When you have completed your evaluation, click SUBMIT located at the bottom of the form. Once you have submitted your evaluation you will not be able to go back to make any additions or changes.
