3M Matching Gifts Instructions

IF YOU NEED ASSISTANCE
To register online for a matching gift from 3M, you do not need to own a computer – you just need access to a computer. Public libraries, family members, or friends can lend a hand. For assistance, you can contact the 3M Foundation at 651/733-0144, and they can put you into contact with 3M retiree volunteers who routinely assist with this process.

I.	Log on to www.3M.com/matchinggifts.
	1.	Follow the links to matching gifts for higher education.
	2.	Enter your 8-digit employee number and password.
	3.	For first-time users, set up your personal 3M Matching Gifts Account using the following instructions:
			Enter your 8-digit employee number (see examples on "Employee Number Help" on the log-in screen).
			Enter your e-mail address, as you will receive an electronic message to confirm your password. If you do not have an e-mail address, use tempmg@3M.com [Note: You will not be able to access this temporary account; it allows you access only to the online system. The 3M Matching Gifts Program administrator will NOT monitor this account, and mail is purged periodically.]
II.	Select the organization name. St. Catherine University is listed by 3M as:
		COLLEGE OF ST. CATHERINE (we'll be changing our name with 3M to "St. Catherine University" soon) Tax ID Number: 41-0695509
III.	Fill in the required information. If you are donating to more than one designation, each designation and specific gift amount will need to be registered separately.
	1.	Enter the Gift Amount. If you are making a gift of stock, please fill in the estimated amount of your gift. The University will confirm your matching-gift registration with the actual gift amount.
	2.	Enter the Gift Type of "Higher Education."
	3.	Enter the Designation for your gift. PLEASE NOTE: Effective January 1, 2004, all participants in 3M's Matching Gift Program will have their donation automatically designated to the "3M and Alumnae Scholarship Fund" unless they note "Annual Fund" (which is the University's "General Fund") or some restriction in the Designation field.
			If your gift is unrestricted, enter "Annual Fund." (Donors giving $1,500 or more to the Annual Fund each year are recognized as members of the President's Forum.)
			Only if you want your gift and the match to go to the "3M and Alumnae Scholarship Fund" should you leave the designation field blank.
	4.	Enter your Gift Payment Method.
	5.	If you donated stock or mutual fund shares, fill in the:
			Number of Shares
			Stock Symbol/Security Name (for example "MMM" or "3M Company")
IV.	Print two copes the “3M Matching Gifts Program Coupon,” keeping one for your records and mailing the other with your donation to the University in a Development Office return envelope or, if you do not have an envelope, to the following address:

		Alternately, you may mail or fax the “3M Matching Gifts Program Coupon” to the University using one of the following:

Effective January 1, 2009, the 3M Foundation will match employee gifts 1:1 up to $10,000 and retiree gifts from those who retired within the last five years at 0.50:1 up to $10,000 (for a maximum match of $5,000) Effective January 1, 2009, 3M Foundation will not match gifts from those retirees who retired more than five years ago.
