On Line Forms

On line forms are an essential part of every department application process. Included in this site is the Dietetic Internship application (2005), Dietetic Internship application (2004) and Dietetic Internship application (1998). This enables students within the dietetic major to apply for dietetic internships. Upon completion of the internship, students may take the registered dietitian exam to become a registered dietitian.


The FCNS (Family, Consumer and Nutritional Sciences) Department strives to enable students to succeed in all aspects during their time here at St. Kates. We offer several scholarship opportunites which many of our majors are eligible to apply for. The application form is due in March of each academic year.


Prior to the end of the fall semester, petition to major forms are due for those wishing to declare their major within the FCNS Department. Being accepted into the department enables the student to apply for department scholarship scholarships and be considered for acceptance into Kappa Omicron Nu (KON), the department honor sciety.
